Role: Travel or local contract · 13 weeks · 40 hours/week
Shift: 8-hour day shifts
Expected dates: Nov 9, 2026 – Feb 6, 2027

Compensation: Approved W-2 or travel pay package shared by recruiter after verification.
Responsibilities
- Provide physical therapy assistant services as directed under the supervising physical therapist's plan of care
- Support patient mobility, function and prescribed rehabilitation activities
- Document visits and communicate patient progress to the care team
Requirements
- Appropriate Illinois physical therapist assistant licensure or eligibility
- Home health physical therapy assistant experience
- Strong patient communication skills
- Ability to meet home health visit documentation and scheduling expectations
