Role: Local contract only · 13 weeks · 40 hours/week
Shift: 8-hour day shifts
Expected dates: Nov 16, 2026 – Feb 15, 2027

Responsibilities
- Assess mobility and rehabilitation needs in a home health setting
- Plan, deliver and document physical therapy treatment
- Coordinate patient progress and visit scheduling with the care team
Requirements
- Appropriate Illinois physical therapist licensure
- Prior home health experience
- Strong time management and caseload organization skills
- Local eligibility under the requisition's candidate policy
